Forman, Oscar-winning director of 'Cuckoo's Nest' and 'Amadeus', dies at 86

Here are key dates in the life of Czech-born United States film director Milos Forman, who died on Friday aged 86.

"One Flew Over the Cuckoo's Nest", in which a psychiatric institution becomes a microcosm of the contemporary world, and "Amadeus", the life of 18th-century composer Wolfgang Amadeus Mozart through the eyes of his rival Antonio Salieri, earned 13 Oscars between them, including those for best director to Forman.

In August 1968 Czechslovakia was invaded by Warsaw Pact forces aiming to suppress Alexander Dubček's liberalising reforms; Forman opted to stay in the USA, were he was joined by fellow director Ivan Passer. However, it took him two years to become a USA citizen.

Forman's other notable films include The People vs Larry Flynt (1996) and Man On The Moon starring Jim Carrey as Andy Kaufman (1999).

Valmont, Forman's 1989 adaptation of Les Liaisons Dangereuses, proved something of an anticlimax as another version, directed by Stephen Frears and containing a star-making performance from John Malkovich, had been released the previous year.

"A director is little bit of everything, little bit of the writer, little bit of an actor, little bit of an editor, little bit of a costume designer", Forman once said.

Never prolific, Forman's output slowed even more after "Amadeus", and his three subsequent films were disappointments.

He was father to two sets of twin boys, with his second and third wives. They also had twin sons.
