A spokesperson for Hacienda HealthCare did not immediately return Fox News' request for comment Tuesday.

The company Hacienda HealthCare told the New York Times that the resignation of executive Bill Timmons was approved by the entire board of directors.

In response, the Arizona Department of Economic Security and Phoenix police launched an investigation into the facility. It is also unclear if staff members at the Hacienda de Los Angeles facility were unaware of the pregnancy until the birth.

For example, an investigation in 2013 found that a staff member had made "inappropriate, sexual statements" about four clients.

Local news website Azfamily.com first reported the woman, who had been in a vegetative state for more than 10 years after a near-drowning, delivered a baby on December 29.

Hacienda HealthCare board member Gary Orman said the facility will accept "nothing less than a full accounting of this absolutely horrifying situation, an unprecedented case that has devastated everyone involved, from the victim and her family to Hacienda staff at every level of our organization".

The facility provides intermediate care for patients with intellectual disabilities. We are already conducting a comprehensive internal review of our processes, protocols, and people to ensure that every single Hacienda resident is as safe and well cared for as possible.

The woman said not one person reported the incident out of fear, adding that it is likely other forms of abuse took place over the years.

The Arizona Department of Health Services (DHS), which licenses the facility, said they were aware of the alleged incident and were "actively working with local law enforcement in their criminal investigation".
